The pillow comes first because height changes how your neck lies
Your pillow needs to fill the space beneath your head without pushing it upwards or letting it drop. Too high and your head tilts up; too low and it drops down. Either way, you spend hours with your neck bent. Buying the fullest-looking pillow in the shop is a surprisingly poor selection method, despite how convincing it looks in a freshly made bed. Judge the height with your head resting on it, because a soft pillow compresses under you.
In their 2021 review of pillow-height research, Lei and colleagues found that pillow height affects cervical alignment, meaning how your neck lies, and the load on neck and shoulder muscles.1 The reviewers found no established optimal height because the evidence is not there yet. This was a review, not a trial. I take that as a reason to match the pillow to your shoulders and sleeping position instead of buying whatever has the most elaborate neck-shaped packaging.
A 2025 study by Tian and colleagues adds a small piece of evidence for side sleepers.2 Nine healthy adults lay on foam-latex pillows at four heights, with and without neck support. Their measured neck curve tracked a height figure based on shoulder width, and a medium individualised height came out best. Nine healthy people, one lab. That gives you a starting point for considering shoulder width, not a formula for choosing a pillow.
I went cheap on pillows for years until I tried a Dunlopillo, and it's been my pillow ever since. The difference in comfort was enough to end that particular false economy. Your version needs to suit how you lie, though; buying the same name as someone else skips the important bit.
Use your sleeping position to choose a height to try. These are comfort starting points, with no universal pillow measurement.
| How you sleep | What to try | What to check while lying down |
|---|
| On your side | A higher pillow that fills the gap between your ear and shoulder | Your head continues the line of your body without tipping towards the mattress or ceiling |
| On your back | A lower pillow | Your head stays level instead of being pushed forwards |
| On your front | A thin pillow if you cannot change position | Your neck still has to turn; changing the position is the better aim |
Check your pillow again when you change mattresses. If your shoulder sinks further into the new surface, the gap beneath your head changes. The pillow that fitted your old bed might then be too high, even though you haven't changed anything else.
Your mattress needs to support your waist and let your shoulder in
Your mattress needs to keep your spine level from the pelvis upwards, with enough give beneath your shoulder when you sleep on your side. A sag beneath your middle changes that line however carefully you've chosen the pillow. This is why I won't send you straight to the hardest bed in the showroom. You already own a floor; hardness alone is hardly a selling point.
My reading of the NHS's firm-mattress advice is supportive: your waist held and your spine level, with enough give for your shoulder. A paving-slab feel presses against a side sleeper's shoulder and can leave the neck bent upwards. That's my view on translating the advice into a shopping decision. Test whether you stay level and feel supported before getting attached to the firmness label.
For side sleeping, I'd start with a hybrid or pocket springs beneath a deep comfort layer. The upper layer gives your shoulder room to sink, while the support underneath holds your middle. If you sleep on your back, pay particular attention to support around your waist and whether your pelvis sinks too far. A soft surface and a supportive mattress can exist in the same bed, which really shouldn't be such a revelation.
Start with your build, then adjust for your sleeping position. A heavier person compresses the same mattress more than a lighter person, so the word "medium" tells you very little by itself. I favour a softer feel for side sleeping than for back sleeping, provided your middle stays supported.
These are my shopping rules of thumb, not clinical thresholds or firmness prescriptions.
| Your build and position | Direction to try | The check that matters |
|---|
| Lighter, especially on your side | A softer feel with room for your shoulder | You sink enough at the shoulder to lie level |
| Heavier | A firmer support core | Your pelvis stays supported without sinking below the rest of you |
| Side sleeper at any build | A little more give in the comfort layer | Your shoulder settles into the surface without your waist dropping |
| Back sleeper at any build | A feel that supports your waist | Your head and body stay level with the lower pillow you use |
If your shoulder also feels pressed against the surface, compare the comfort-layer guidance for mattresses for shoulder pain. More cushioning is useful when the surface is too hard, but adding cushioning over a sag leaves the sag underneath.
Your sleeping position and bed base are worth checking before you spend
Check how you're lying and what is underneath the mattress before replacing the bed. A different pillow height costs less to try, and a broken slat can leave a dip beneath an otherwise usable mattress. Neither check is especially glamorous. Still, I prefer a slightly dull inspection to an expensive purchase that leaves you with the same problem.
Front sleeping is the position I'd talk you out of for neck comfort. You have to turn your head to breathe, so even a thin pillow leaves your neck twisted. Try settling on your side or back instead. If you cannot change position, keep the pillow thin so you aren't adding more height beneath an already turned head.
On your side, check that your head follows the line of your body instead of tilting down into the pillow or up away from it. You need enough pillow beneath your head and enough give beneath your shoulder. The guidance on mattresses for side sleepers is useful when the shoulder space is the part your existing bed cannot provide.
At home, try a different pillow height before committing to a mattress. Keep the rest of the bed the same while you compare, so you can tell what changed. My housekeeping view is to replace pillows every couple of years, with an earlier look if they've flattened or become lumpy. Loyalty is lovely in a relationship; you don't owe it to an elderly pillow.
Lift the mattress and inspect the base if you can do so safely. A broken slat or dipped base reduces support and can change how your whole body lies overnight. Check the mattress itself for a lasting dip as well. A topper can soften an overly firm surface, but it cannot restore the support beneath a hollow.
Test the pillow and mattress together before you commit
Choose the pillow height first, then test mattresses with a pillow that feels like the one you'll use at home. The shop's enormous display pillow is excellent at looking expensive and less useful for this particular job. Give yourself at least ten minutes lying down, including time on your side and back, with most of it spent in your usual position.
Ask someone with you to look at whether your head stays level with your body. On your side, check that your shoulder sinks into the comfort layer and your waist remains supported. On your back, notice whether your middle drops or your head is pushed forwards. A quick sit on the edge tells you almost nothing about any of that, however efficiently you usually shop.
Try a hybrid or pocket-sprung mattress with a deep comfort layer first if you're unsure where to begin. Turn over as well as lying still. If you move frequently, you need to be able to change position without struggling out of deep foam. If you also run hot, I think deep memory foam is a poor match; its close-fitting feel can be warm.
Read the returns terms before paying. Check whether a comfort return means a refund or an exchange, what condition the bed must remain in, and whether collection costs you anything. A home trial is useful only if you understand how to use it. Save the terms somewhere you can find them without conducting a midnight search through your inbox.
Allow for some adaptation during the first fortnight: the feel on the first evening needn't be the feel you settle into. Write down which pillow you're using and how comfortable you feel on waking, because a short note is more useful than trying to remember several mornings at once. If neck pain persists for a few weeks, or you're worried about it, contact your GP instead of extending the mattress experiment.
Avoid replacing the whole bed before trying the pillow
A new mattress is an expensive first experiment when the pillow height is wrong. I know a bed delivery feels more decisive than changing a pillow, but the smaller purchase is the sensible place to begin. These are the choices I'd leave out of your shopping plans:
- A new mattress before trying a different pillow. Height is the cheaper thing to change, and it directly affects how your head rests.
- A high, soft pillow that lets your head roll. A tall pile of filling is little use if your head drops or tilts once you lie on it.
- Sleeping on your front with neck pain. Your head stays turned, even with a thin pillow.
- Deep memory foam if you turn frequently and run hot. Choose a surface you can move across easily without the close, warm feel.
- A mattress purchase as the answer to pins and needles or a cold arm. Contact your GP about those symptoms.
Ignore "orthopaedic" on a pillow or mattress when deciding whether it fits you. It doesn't establish the right height or the right support for your body. I'm unimpressed by medical-sounding embroidery; check how you actually lie on the bed.
When neck pain needs a GP, not a new pillow
Some neck pain needs medical assessment rather than a different bed. NHS guidance on neck pain and stiff neck says to see a GP if neck pain or stiffness does not go away after a few weeks, if painkillers like paracetamol or ibuprofen have not worked, if you're worried about the pain, or if you have other symptoms, like pins and needles or a cold arm.
For sleeping with neck pain, the NHS advises using a low, firm pillow, making sure your head is the same height as the rest of your body, not sleeping on your front, and not twisting your neck when you're in bed.
